I complained in May about Hamilton Sexual Health dosing my oestrogen as to make me really sick and was absolutely disgusted by the response. Telling me you're "sorry I feel that way" while telling me it was my fault for not begging for more when I was trusting your judgement and had no idea low oestrogen was causing my symptoms is absolutely appalling behaviour from a medical doctor. I repeatedly complained about feeling extremely sluggish and emotionally flat, as demonstrated in my medical notes. My blood tests, when you bothered to start testing 8 years later, showed I had oestrogen levels far below the levels typically recommended by Dr. Samantha Newman at Female GP to treat menopause (200 to 300 pmol/L), let alone recommended by both the Endocrine Society (2017) and WPATH (367 to 734 pmol/L, first published on 13 Sep 2017 and I was a patient at Sexual Health for the next five years), yet you repeatedly ignored it. Suspiciously you only started testing when care was handed over to my GP. It's almost like you were trying to hide it. Why do my notes state that you asked if I was depressed eight times, offered counselling three times, and noted I was taking antidepressants nine times, if nothing was wrong? Why would an organisation of professional endocrinologists recommend much higher oestrogen levels if it were unsafe or unnecessary? Why do you, as a non specialist, think you know better? Just because I said I thought fatigue was work related and didn't recognise anhedonia as depression doesn't excuse Hamilton Sexual Health. You lied and told me first that it was just normal for women to feel tired all the time, that all your other patients felt the same way, and that it was due to women having lower haemoglobin. You even mentioned I had fatigue multiple times in your response, which might I add never resolved. You lied and said I was getting really good results, manipulating me into not asking for more. You left me unable to concentrate and struggling to work, despite having previously been an honours student in accounting. I struggled with overeating, gaining a lot of weight, especially belly fat; I felt hopeless and like giving up on life, and was always cranky and irritable. I was so tired I struggled to take showers and to make my own bed. I even had frequent night sweats. You people are medical doctors and need to take responsibility for the health and wellbeing of your patients. Just because the guidelines allow you to dose me to literally zero doesn't give you the right to make me sick. Dr. Karen Benattar also gave false advice to my GP. The reference range for testosterone at PathLab is based on the 95th percentile, are intended for diagnosing medical conditions, and are inappropriate to use for gender affirming care. The NZ guidelines clearly state the upper limit for testosterone is 2.0 nmol/L, with the Endocrine Society recommending between 0.3 and 1.7 nmol/L. The guidelines also state that if oral oestrogens are too risky the advice is to switch to transdermal not just let someone be miserable. References Hembree, W. C., Cohen-Kettenis, P. PSA for all transgender women: Beware of low-dosing! These people have an extremely out-dated understanding of hormones. Your blood tests should have you in the 300-370 pmol/L range for Oestradiol/Estradiol. (80-100pg/mL) Having low levels of Estradiol will put you in the pre-menopausal range - which comes with symptoms such as fatigue, dizziness, dry skin, and will even put you at risk for Osteoporosis. I have been on disability benefits for more than 10 years because of this. A simple Google search of [any] transgender guidelines for ideal Estradiol levels will reveal that I am not making this up. Take care of your health, do your blood tests, and have a great day. P.s. Do not take more than 12.5mg of Cyproterone Acetate once you've been on hormones for more than 6-12 months.
